Back Carrara doesn't support NURBS-although the Spline Modeller sort of looks like one-so everything you do in the Spline Modeller gets converted to polygons. To make your spline models smoother you can increase the surface fidelity setting which should produce more polygons to more closely approximate your spline curve. In a worst case scenario you could try converting it to a Vertex model and make it a subdivision surface but that's probably asking for trouble.
